2021 Guest Waiver Form

All guests and non-members who attend an activity, trip, or program with a fee attached and advance sign up required, must have a Guest Waiver form on file.  This is so that we are able to reach you or a family member if needed while you are participating with the Senior Center.  You can download the form below, fill it out online, print it, sign it, and return it to the Castle Rock Senior Activity Center by the time of the event you are attending.  The form is valid for the calendar year.  Thank you for your attention in this important matter.
